How Does CBD Work? The Real Story Behind Topical CBD for Pain Relief
By James Palmer, CMT — The Massage Guy
This is one of the most popular questions I get from clients. Everyone wants to know: does CBD actually work, or is it just expensive snake oil?
Here's my take: CBD is great and can help in certain situations. It's not a magic potion on its own — the CBD component specifically works behind the scenes rather than giving you that immediate sensation you might expect. But here's where it gets interesting: the CBD cream I use (Cause & Medic Relief Cream) actually combines CBD with menthol and capsaicin, so you get both the immediate topical relief AND the deeper, longer-lasting benefits of CBD working with your endocannabinoid system.
Let me break down what's actually happening when we use this stuff.
The Endocannabinoid System: Your Body's Built-In Regulation Network
Your body has something called the endocannabinoid system (ECS) — a network of receptors found throughout your body that help regulate a variety of functions like sleep, pain perception, inflammation, and immune system response.*
Think of the ECS as a communication network that helps your body maintain balance. It's involved in everything from how you experience pain to how your muscles recover after exercise. This system exists whether or not you've ever used cannabis products — it's a natural part of your biology.
When CBD is applied topically, it interacts with cannabinoid receptors in the skin (specifically CB1 and CB2 receptors), activating the endocannabinoid system locally and potentially providing relief for muscle tension, inflammation, and discomfort.*
What I've Seen in My Practice
While my clients have not reported immediate pain relief, per se, it does seem to help relax overly tight muscles and provide the ability to work the tissue a little deeper during deep tissue massage or sports massage sessions.
Here's what typically happens: I apply the CBD cream to an area of chronic tension — let's say tight shoulders from desk work, or a stubborn knot in the lower back. After a few minutes, the tissue seems to respond differently. It's not that the pain disappears instantly; it's that the muscle becomes more receptive to the work. Clients can tolerate deeper pressure more comfortably, and the tissue releases more effectively.
This aligns with what research suggests about CBD's anti-inflammatory and muscle-relaxing properties when applied to the skin.** The cannabinoids interact with receptors right where you apply them, helping to modulate the local inflammatory response and reduce muscle tension.
Topical vs. Oral: Different Delivery, Different Effects
It's important to understand that taking CBD orally affects the body in a completely different way than applying it topically.
When you take CBD orally (like gummies, tinctures, or capsules), the CBD enters your bloodstream and travels throughout your body, eventually reaching the brain and other cannabinoid receptors. This systemic approach would have more of an effect on sleep, mood, anxiety, and overall immune function.
When you apply CBD topically — like we do during massage — it generally only affects the area where it's applied. It doesn't necessarily enter the bloodstream in significant amounts; instead, it attaches to the cannabinoid receptors in the skin and underlying tissue in that specific location.
This is actually ideal for massage therapy, because we're targeting specific areas of tension and pain — tight shoulders, lower back pain, hip flexor tightness from sitting all day. The localized application means the CBD is working exactly where you need it, without systemic effects.
What to Expect During a CBD-Enhanced Massage Session
If you want to give CBD pain relief cream a try during your next session, here's what to expect with Cause & Medic Relief Cream:
The Immediate Effect (Menthol & Capsaicin):
Cooling sensation from menthol — You'll feel this pretty quickly, which helps with immediate pain relief and creates a soothing sensation on the skin
Warmth from capsaicin — This kicks in shortly after and helps increase blood flow to the area, which aids in muscle relaxation
Surface-level pain relief — These ingredients work as counterirritants, essentially distracting your pain receptors
The Deeper Effect (CBD):
Gradual muscle relaxation — Over 5-10 minutes, as the CBD interacts with cannabinoid receptors in the tissue, muscles tend to become more pliable and less guarded
Reduced inflammation — The CBD component works on a cellular level to help calm the inflammatory response
Deeper work with less discomfort — The combination means I can often work more effectively on stubborn knots and tight areas
Longer-lasting effects — Many clients report that the relief lasts longer than sessions without CBD, even after the menthol/capsaicin sensation fades
So you're getting the best of both worlds: immediate topical relief from the menthol and capsaicin, plus the therapeutic benefits of CBD working with your body's endocannabinoid system.
Cause & Medic Relief Cream is specifically formulated for therapeutic bodywork, combining all three ingredients in concentrations that are effective without being overwhelming. If you'd like to add this to your next mobile massage session in Los Angeles, just let me know when you book.
Who Benefits Most from CBD During Massage?
CBD-enhanced massage tends to work particularly well for:
Chronic muscle tightness that hasn't responded well to regular massage alone
Inflammatory conditions (like tendonitis or bursitis)
Athletes recovering from intense training or competition
People with limited range of motion due to muscle guarding
Desk workers with persistent neck, shoulder, and upper back tension (this is basically everyone in Beverly Hills and West Hollywood)
The Bottom Line
CBD isn't a miracle cure, but it's a legitimate tool that can enhance the effectiveness of therapeutic bodywork. The science is still evolving, and we need more research to fully understand all of its mechanisms and applications. But based on what I've seen working with clients throughout Santa Monica, Hollywood, Calabasas, Studio City, and The Valley, I can say this: when used correctly as part of a comprehensive treatment approach, topical CBD can make a real difference.
It's important to note that while CBD may have potential benefits, it's always important to speak with a healthcare professional before using any new product, especially if you have a medical condition or are taking medications.
